In TaxSlayer Pro, some Individual Tax Return (Form 1040) forms, schedules, and worksheets that are listed in the Blank Forms menu are not listed in the Form Finder within the return.
Form 1040-PR
To produce a Form 1040-PR, begin the return as a Form 1040 with the taxpayer’s address in Puerto Rico. Then, from the main menu of the tax return (Form 1040) select:
- Personal Information
- Other Categories
- Bona Fide Resident of Puerto Rico – Answer YES.
- Print Return on Form 1040PR/1040SS – Choose 1040-PR.
Form 1040-SS
To produce a Form 1040-SS, begin the return as a Form 1040 with the taxpayer’s address in Puerto Rico, Guam, US Virgin Islands, or the Commonwealth of Northern Mariana Islands. Then, from the main menu of the tax return (Form 1040) select:
- Personal Information
- Other Categories
- Bona Fide Resident of [territory] – Answer YES.
- Print Return on Form 1040PR/1040SS – Choose 1040-SS.
Form 1040-SR
Form 1040-SR, U.S. Tax Return for Seniors, is produced automatically if the primary taxpayer or spouse is over the age of 65. If it is included in the return, it will appear under View Results and will print with the rest of the return instead of Form 1040.
Form 2120
To access Form 2120, Multiple Support Declaration, from the main menu of the tax return (Form 1040) select:

Form 2350
Form 2350 is used to request an extension of time to file. It is the equivalent of Form 4868 for a taxpayer who (a) is living abroad, and (b) expects to file Form 2555 and needs time to meet either of the two tests (bona fide resident or physical presence) to qualify for the foreign earned income exclusion and/or the foreign housing exclusion or deduction.
To indicate that you wish to file an extension request on Form 2350 rather than Form 4868, from the opening screen of TaxSlayer Pro select:
- The Extensions icon at the top of the screen (labeled 4868 prior to 2019)
or - E-file
- Automatic Extension Application (Form 4868)
- Enter the taxpayer SSN and answer the confirmation and filing status questions.
- In the Extension Personal Information window, select the button next to “2350 (Qualifying Taxpayers Abroad)”.
Form 4852
Form 4852 serves as a substitute for either Form W-2 or Form 1099-R, and it is necessary when the payer hasn’t issued one of those forms or issues an incorrect form.
To indicate that W-2 information entered in the return is obtained from a Form 4852, from the main menu of the tax return (Form 1040) select:
- Income
- Wages, Salaries, Tips (W-2)
- New
- Select the check box next to h) Substitute and answer YES
To indicate that 1099-R information entered in the return is obtained from a Form 4852, from the main menu of the tax return (Form 1040) select:
- Income
- IRA/Pension Distributions (1099R,RRB-1099-R)
- New
- Select the Substitute check box on Line g and answer YES
Form 5884-A
Form 5884-A, Credits for Affected Disaster Area Employers (For Employers Affected by Hurricane Harvey, Irma, or Maria or Certain California Wildfires) is used by taxpayers to claim the employee retention credit. This is a general business credit, and the current year amount, a carryforward, or a carryback can be entered by selecting from the main menu of the tax return (Form 1040):

General Business Credit Carryforwards, or
General Business Credit Carrybacks - Form 5884-A
Form 6198
Form 6198, At-Risk Limitations is produced when
- there is a loss on Schedule C, E, F, or K-1, and
- the investment in the business activity is not all at risk.
For a particular schedule where both of these conditions are true, when exiting the menu item for the business you will pass through the “Form 6198 – At-Risk Limitations Menu” where you can complete the appropriate lines on the form as needed. In addition, a Form 6198 menu line is available as needed for each property on Schedule E.
Form 6251 Worksheets
Form 6251 Exemption Worksheet for Line 5 is included in the Form 6251 instructions. It is produced as needed based on information provided elsewhere in the tax return. To view the worksheet, from the main menu of the tax return (Form 1040) select:

Form 8332
To access Form 8332, Release/Revocation of Release of Claim to Exemption for Child by Custodial Parent, from the main menu of the tax return (Form 1040) select:

Form 8453
Form 8453 is required for an e-filed individual tax return when certain supporting documents are required by the IRS. The form will need to be printed and mailed to the IRS along with the required documents as specified on the form. The form is generally produced automatically with the check box next to the relevant form(s) checked, but there are four check boxes on the form that may need to be manually checked so that the form can be produced. To access Form 8453 for these four check boxes, from the main menu of the tax return (Form 1040) select:

Form 8582
Form 8582, Passive Activity Loss Limitations, is automatically produced if it’s needed, as the entries on most of the form are pulled from information already existing elsewhere in the return. To access the report for the few items that may need to be entered manually, from the main menu of the tax return (Form 1040) select:

- Select the line needing an entry:
- Gain reported on 4797 from a Rental Disposition – entered on Form 8582 Worksheet 1

Form 8878
Form 8878 is the signature form for e-filed extension requests. See here for instructions on how to create an extension. To print Form 8878, from the Form 4868 Extension Menu select
- Send Extension Electronically? – change answer from NO to YES

- View icon in the toolbar at the top of the screen (a black magnifying glass prior to 2019)
Form 8888
Form 8888 is used in e-filed returns to split the refund between a direct deposit and a check, to allocate the refund to two or three bank accounts, or to buy U.S. Series I Savings Bonds (in $50 increments). This form cannot be used in conjunction with a bank product. In TaxSlayer Pro, purchase of a Savings Bond requires at least a portion of the refund to be direct deposited. To access Form 8888, from the main menu of the tax return (Form 1040):
- Select Mark Return Electronic
- If queried whether or not the taxpayer wants fees deducted from the refund, answer NO.
- Select Direct Deposit – Not a Bank Product
- Enter the bank routing number, account number, and account type.
- Complete the Practitioner Pin information as appropriate.
- From the E-File Menu, select Form 8888
- Select Amount to be Deposited in First Account, and enter the amount. This amount will be deposited to the first bank account already entered.
- If a second direct deposit is desired, select Amount to be Deposited in Second Account, and enter the bank info along with the amount.
- If a third direct deposit is desired, select Amount to be Deposited in Third Account and enter the bank info along with the amount.
- If the taxpayer wishes to buy U.S. Savings Bonds, select the appropriate Savings Bond line and enter the information. Bonds are purchased in $50 increments.
- If any remainder is desired to be received in the form of a check, select Amount to be Refunded on a Check.
Form 8907
Form 8907, Nonconventional Source Fuel Credit, expired in 2013 and is only available in the General Business Credits as a carryforward. To access Form 8907, from the main menu of the tax return (Form 1040) select:

Form 8931
Form 8931, Agricultural Chemicals Security Credit, expired in 2012 and is only available in the General Business Credits as a carryforward. To access Form 8931, from the main menu of the tax return (Form 1040) select:

Form 8959
Form 8959, Additional Medicare Tax, is produced as needed based on information provided elsewhere in the return. To see the amount of additional Medicare tax calculated, from the main menu of the tax return (Form 1040) select:

Form 8997
Form 8997, Initial and Annual Statement of Qualified Opportunity Fund (QOF) Investments, is used by an investor in a QOF to inform the IRS of the following:
- QOF investments and deferred gains held at the beginning and end of the current tax year;
- Capital gains deferred and invested in a QOF;
- QOF investments disposed of during the current tax year.
The sale of a QOF investment is generally reported on Form 1099-B. To access Form 8997, from the Main Menu of the tax return (Form 1040) select:

Form 9325
The information in Form 9325, Acknowledgement and General Information for Taxpayers Who File Returns Electronically, is obtained from the acknowledgements received subsequent to a tax return or an extension request being e-filed. The form can be produced for one taxpayer or for all taxpayer acknowledgements within a given date range. To access Form 9325, from the opening screen of TaxSlayer Pro select:

Additional EIC Due Diligence
If a return includes the Earned Income Credit, you can enter additional notes documenting your due diligence efforts. From the Main Menu of the tax return (Form 1040) select:

The Additional Due Diligence Documentation will print with the rest of the return.
Bonus Depreciation Opt Out Election
When entering an asset in the Depreciation Module, you can indicate that you are opting out of bonus depreciation for the asset. This form will be included in View Results for any assets so indicated.
Capital Gain Tax Worksheet
The Qualified Dividends and Capital Gain Tax Worksheet is in the Form 1040 instructions and is produced as needed based on information entered in the return.
Clergy Worksheet
The Clergy Worksheet is included in IRS Publication 517 and is related to the Schedule C for a self-employed member of the clergy. To make entries in the worksheet, from the Main Menu of the tax return (Form 1040) select:

Dependents Standard Deduction Worksheet

Depreciation Worksheet
This worksheet is produced as needed if depreciable assets are included in Schedules C, E, or F.
Head of Household Worksheet
This due diligence worksheet is gleaned from information included in IRS Publication 501 and is produced for Head of Household filers if TaxSlayer Pro has been configured to produce it for Head of Household filers. In the return, it will be available to print under View Results, either by itself or in a print set. To indicate that you would like this worksheet to always be produced in Head of Household filer returns, from the opening screen of TaxSlayer Pro select:

The questions on the Head of Household Worksheet are asked when the filing status of Head of Household is chosen in a return. Only as many questions are asked as are needed to determine whether or not the taxpayer qualifies.
IRA Worksheet
The IRA Deduction Worksheet in included in the Form 1040 instructions and is produced as needed based on information entered in the return.
Schedule A Mortgage Interest Worksheet
Worksheet To Figure Your Qualified Loan Limit and Deductible Home Mortgage Interest For the Current Year is in IRS Publication 936 and is produced as needed based on information entered in the return.
Schedule A Contribution Worksheet
This worksheet is included in IRS Publication 926 and is produced as needed based on information entered in the return. It isn’t used if Schedule A includes a carryover of a prior year charitable contribution.
Schedule A Limitation Worksheet
This worksheet was in the instructions for Schedule A (Form 1040) prior to 2018. It is obsolete for tax years after 2017.
Schedule D Tax Worksheet
This worksheet is found in the instructions for Schedule D (Form 1040) and is produced as needed based on information entered in the return.
Schedule D Capital Loss Carryover
The Capital Loss Carryover Worksheet is in the instructions for Schedule D (Form 1040) and is produced as needed based on information entered in the return.
Schedule D 28% Rate Worksheet
The 28% Rate Gain Worksheet is in the instructions for Schedule D (Form 1040) and is produced as needed based on information entered in the return.
Schedule E IRC Sec 469(c)(7)(A)
Taxpayers who are real estate professionals can elect to treat all their rental activities as a single activity rather than separate activities under IRC Section 469(c)(7)(A). To make this election in TaxSlayer Pro, from the Main Menu of the tax return (Form 1040) select:
- Income
- Schedule E
- IRC Section 469(c)(7)(A) Election – answer YES to the question. This will produce an election statement in the return.
Note: This election continues beyond its initial year and can only be revoked in a year when the taxpayer’s facts and circumstances have changed. This menu item is only used to make the election. Revoking the election is done by creating a preparer note to that effect in the return under Notes / Statements.
SEP Contribution Worksheet
The Deduction Worksheet for Self-Employed is included in IRS Publication 560. To access the worksheet in TaxSlayer Pro, from the Main Menu of the tax return (Form 1040) select:

Simplified General Rule Worksheet
The Simplified Method Worksheet is included in the Form 1040 instructions and must be used to figure the taxable part of a pension or annuity for either of these two situations:
- The annuity starting date was after July 1, 1986, and the taxpayer used the Simplified Method last year to figure the taxable portion;
- The annuity starting date was after November 18, 1996, and both of the following are true:
- The payments are from a qualified employee plan, a qualified employee annuity, or a tax-sheltered annuity;
- On the annuity starting date, either the taxpayer was under age 75 or the number of years of guaranteed payments was fewer than 5.
The worksheet is needed if the SGR box is checked on a Form 1099-R. In the Form 1099-R input screen, the SGR check box is on Line 2a, Taxable Income, and after checking the box you will be asked if you would like to use the Simplified General Rule Worksheet. Answer YES, and enter the requested information as needed.
SSA Worksheet
There are four Social Security benefit worksheets included in IRS Publication 915, and they will be produced in a tax return as needed based on the information entered. To enter Social Security benefits in TaxSlayer Pro, from the Main Menu of the tax return (Form 1040) select:
